Hydrochlorothiazide

PRODUCT DESCRIPTION
Yellow, round and flat tablets, 8mm diameter with score-line and without logo.

COMPOSITION
Hydrochlorothiazide BP 50mg/tablet.

PHARMACOLOGY
Hydrochlorothiazide is a diuretic which reduces the reabsorption of electrolytes from renal tubules, thereby increasing the excretion of sodium and chloride ions, and consequently of water. It also has a slight lowering effect on the blood pressure and it enhances the effects of other antihypertensive agents.

PHARMACOKINETICS
Hydrochlorothiazide is absorbed rapidly from gastrointestinal tract with a bioavailability of about 65 to 75%. It excreted mainly unchanged in urine and has a terminal half-life of 6 to 15 hours.

INDICATION
For oedema and hypertension.

RECOMMENDED DOSAGE
Adults: For hypertension: 25 to 100mg daily in single or divided doses. For oedema: Initially: 50 to 100mg 1 to 2 times daily; maintenance: 25-50mg on alternate days.

SIDE EFFECTS
Skip rashes, thrombocytopenia, thirst, epigastric pain, anorexia, constipation, gastric irritation, nausea, vomiting, diarrhoea, dizziness, headache, weakness, photosensitivity, postural hypotension, impotence and yellow vision.

CONTRA-INDICATIONS
Anuria; patients receiving lithium therapy. Patients hypersensitive to hydrochlorothiazide.

PRECAUTIONS
May cause hypokalaemia, aggravate diabetes and gout. Caution in patients with renal or hepatic impairment.

DRUG INTERACTIONS
It may enhance the potassium depleting action of corticosteroids or enhance the neuromuscular blocking action of non-depolarising muscle relaxants. Its hypokalaemia effect can increase toxicity of cardiac glycosides.

USE IN PREGNANCY AND LACTATION
Hydrochlorothiazide crosses placental barrier and is excreted in breast milk. It should not be used in pregnant and lactating women unless the potential advantage justifies the possible risk to the foetus.

OVERDOSAGE AND TREATMENT
Common symptoms of overdosage are those caused by electrolyte depletion ( hypokalemia, hyponatremia, hypochloremia ) and dehydration resulting from excessive diuresis. Symptomatic treatment should be given and mainly on fluid and electrolyte replacement. For recent ingestion gastric lavage should be carried out.
