Indications:

Supplementary therapy in moderate & sever hypertension.
Injection
Malignant hypertension in particular, pre-eclampsia & eclampsia associate
with toxaemia of pregnancy, pulmonary edema in cases of hypertensive heart failure to afford rapid symptoms relief from the cardiovascular effects of thyrotoxicosis.
Dosage:
Tab
Initially 10 mg once daily, raised later if required up to 25-50 daily.
Injection
Hypertension 1-2 ampoules
intra-muscular.
Rapid reduction in blood pressure 1 mg/mins
intravenous infusion.
Contraindications:
Phaeochromocytoma; prior treatment with MAO
inhibitors; heart failure not caused by hypertension.
Special Precautions:
Coronary or cerebral arteriosclerosis; history of
gastrointestinal ulceration; renal impairment; asthma; fever; road users. Withdraw prior to surgery. Pregnancy, lactation.
Adverse Reactions:
Mild central nervous system (CNS) disturbances, postural hypotension; diarrhea, parotid gland swelling; asthma; fluid retention. Occasionally, bradycardia, depression. Prolonged use, heart failure; sick sinus syndrome. Disturbances of potency.
Drug interactions:
Combination with antiarrythmics may cause sinus bradycardia. Hypotensive effect potentiated by reserpine, methyldopa, minoxidil & other vasodilators, cal antagonists, β-blockers, ACE inhibitors, alcohol. Tricyclic antidepressants, pizotifen, chlorpromazine, sypathomimetics, Oral
contraceptives antagonize hypotensive effect. Ismelin potentiates hypoglycemic effect of insulin & oral antidiabetics.
